The following Forms and Documents are available for download:

• Index Reconciliation Summary Form
• Cost Transfer Justification Memo
• Journal Voucher Template -  A form similar to this can be created in Excel for use in documenting JVs. Banner entry is then accomplished using the information on this form. The JV number is added when a Banner number is obtained. A copy of supporting documentation is attached to this form, and kept in the department.
